425 cc implants are large, weigh approximately 1 lb
each and will descend inferiorly and laterally requiring revision.Your breasts are too low and the added weight
will require a lift.Many women refuse
lifts because of the ugly vertical scars associated with traditional
techniques.I recommend a new technique
called The Horndeski Method™.The breast
tissue is reshaped creating upper pole fullness, elevated higher on the chest
wall and more medial to increase your cleavage.Aligning the areola and breast tissue over the bony prominence of the
chest wall maximizes anterior projection without the use of implants.If you want implants, they can be placed
simultaneously.I always recommend small
round textured silicone gel implants placed retro-pectoral since they look and
feel more natural, are more stable, less likely to ripple or have complications
needing revision.Implant profile is
irrelevant in the retro-pectoral position since the muscle compresses it.22% of women who undergo breast augmentation
alone required revision within 2 years because they are dissatisfied with the
result.Frequent causes of
dissatisfaction are over augmentation and not performing a lift when

In cases of mild breast ptosis without adequate upper breast tissue I have also inserted implants in a dual plane pocket with good results.  Regarding the implant size, this would be chosen based on your breast dimensions and your preference after trying on sizers, looking at photos etc.  Presumably this was done during your consultation.  I hope you have a great result.

If your ptosis is mild, I agree with your plastic surgeon.  I try not to place scars on a breast unless it is absolutely necessary. Breast lift increases scarring. Ask your surgeon if higher profile implants were used.
