Dual plane natrelle 425 implants? (Photo)

Question

Mild ptosis. The surgoen said if anything he would lift the one but he doesn't recommend it with the scars. He said dual plane natrelle implants and I think he said 425 because I wanted a D cup ( I know bra sizes don't really match up with impany sizes) I tried it On and it looks ok. I'm 22 8 stone 10 and 5ft 5. I have loose thin skin on top of the breast so he said he could not put the implant above the muscles because I would get rippling. I know implants do not lift but will they improve them.

425 cc implants are large, weigh approximately 1 lb
each and will descend inferiorly and laterally requiring revision.Your breasts are too low and the added weight
will require a lift.Many women refuse
lifts because of the ugly vertical scars associated with traditional
techniques.I recommend a new technique
called The Horndeski Method&trade;.The breast
tissue is reshaped creating upper pole fullness, elevated higher on the chest
wall and more medial to increase your cleavage.Aligning the areola and breast tissue over the bony prominence of the
chest wall maximizes anterior projection without the use of implants.If you want implants, they can be placed
simultaneously.I always recommend small
round textured silicone gel implants placed retro-pectoral since they look and
feel more natural, are more stable, less likely to ripple or have complications
needing revision.Implant profile is
irrelevant in the retro-pectoral position since the muscle compresses it.22% of women who undergo breast augmentation
alone required revision within 2 years because they are dissatisfied with the
result.Frequent causes of
dissatisfaction are over augmentation and not performing a lift when

With that said, I honestly think you should not get a lift since your ptosis is really moderate or minimal, with just breast implants you'll look superb.Dual plane location is an excellent option which I have used for 10 years in my career, however the subfascial plane has many advantages that my patients enjoy the latest 7 years, even in slim or skinny patients.Go for anatomical, cohesive gel and macrotextured implants.

In cases of mild breast ptosis without adequate upper breast tissue I have also inserted implants in a dual plane pocket with good results.  Regarding the implant size, this would be chosen based on your breast dimensions and your preference after trying on sizers, looking at photos etc.  Presumably this was done during your consultation. If your ptosis is mild, I agree with your plastic surgeon.  I try not to place scars on a breast unless it is absolutely necessary. Breast lift increases scarring. Ask your surgeon if higher profile implants were used.

Relatively larger implant may correct some skin laxity and glandular ptosis which may very well be appropriate in your case. I'm sure your plastic surgeon recommended the implant based on your breast width and height. If so, you should be able to have a good result. Good luck!

I believe dual plane placement is typically the best, especially for thin women because sub glandular placement can be very obvious and the implant is both visible and palpable. The implants will give you a slight lift so until you need a formal breast lift and can accept the scars, you should be good to go! Good luck

Judging by your photographs and your information this looks like a good surgical plan.When a patient presents with a small amount of breast ptosis (sag) a dual plane breast augmentation is a very effective technique to help avoid the need for a breast lift and still allow the implant to be placed under the muscle.  When an implant is placed under the muscle and the overlying breast has some degree of sag the muscle will hold the implant back and not allow it to fill the loose tissue at the bottom of the breast The word dual means two and the  word plane refers to the natural separation between two layers of tissue.  In the breast there is a natural separation between the breast tissue and the underlying muscle.  During a dual plane augmentation the breast tissue in the lower part of the breast is separated from the underlying muscle.  The muscle is then released along its entire attachment at the bottom of the breast.  The muscle then moves upward like a window shade toward the nipple.  With the restriction of the muscle in the lower part of the breast removed the implant is then allowed to fill the loose sagging part of the lower breast.  This is a very effective technique to help prevent the need for a breast lift in a patient with a small amount of sag and still have a benefits of an implant placed under the muscle.

The lovely thing about the shaped implants is because the point ofa imam projection is lower on the implant, they do tend to swing the nippleup just a bit.   Since you appear more deflated than saggy from your picture, I would also agree with starting with just the implant. You can always do a small lift later if you need it.   As to submuscular versus subglandular I agree with your doctors reasoning and would also use the dual plane approach if I thought your breast tissue were too thin to hide an implant.  I am sorry I can't be more definite, but some things can only be really assessed with an in person exam. I hope this helps, beat of luck.
