I currently have natrelles inspiras 420cc tsx and I am unhappy with how narrow they are as I have a large gap and not much cleavage. My surgeon has agreed to change them to either 560tsf or 615 tsx. I am confused as with the tsf. I will lose projection which I love I don't want to lose any size just gain some width so I have that curvy hourglass shape and full proudbreast with lots of cleavage

But based on your statements above I would recommend an ultra high profile implant at the same or greater base diameter as your current breast size. The ultra high profile would give you the projection that you are looking for. The larger base diameter will give you the fullness in the middle and sides that you currently lack. I recommend finding a boy certify plastic surgeon in your area for a consultation. During the consultation he can measure your breast width and then look at the available implants at your desired size.
