I'm 26 years old. And I'm uncomfortable with the way my tip looking, when I smile it gets wider and from the side droppy. But my tip ifof nose is slightly thick skin and also every morning my nose swollen, and my question is if iI can get the exact look like at the picture.

Hello and thank you for your question. Based on your photograph, you may benefit from a rhinoplasty. A surgeon can accomplish this by trimming, suturing, and reshaping the cartilage in your lower nose. Fascial grafts may also be used to help improve tip refinement. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

In the photos, your natural tip seems moderately broad and rounded, while the second image shows a narrower, more defined appearance. Rhinoplasty may refine a wide or bulbous tip by reshaping and suturing the tip cartilages and, when appropriate, adding structural cartilage support. Drooping with smiling may also be improved in selected patients, but the technique depends on cartilage strength, skin thickness, tip projection, and the muscles that move the nose during expression. Exact replication of an edited or reference photo should not be expected. Thicker skin can limit how sharply the underlying cartilage definition becomes visible, so meaningful improvement is a more realistic goal than perfection. The frontal images are helpful, but standardized side and smiling views would be needed to assess the reported droop and dynamic widening. Morning swelling can also relate to allergies, congestion, or fluid retention and may not respond to rhinoplasty alone. Bring the second image to an in-person consultation as a communication tool and ask the surgeon which aspects are realistically achievable while preserving breathing and facial balance.

What you are describing is very common and very treatable. A tip that drops and widens when you smile usually happens because the muscles around the mouth pull the tip downward, and the tip's natural support allows this movement. With a tip-focused rhinoplasty, the tip can be lifted slightly, refined, and given stronger support so it holds its shape including when you smile. Your slightly thicker skin and morning swelling are normal details that simply need to be factored into the plan; they do not prevent a beautiful result, but they do mean the refinement should be built on good structural support so the improvement actually shows through the skin.About the "exact look" in the second picture I want to give you an honest answer. No responsible surgeon can promise an exact copy of a photo, because your result will always be shaped by your own skin, cartilage, and facial structure. What a well-planned surgery can do is move your nose clearly in the direction of that photo: a more lifted, more refined tip that stays elegant when you smile. A rhinoplasty would be the best option to help achieve your goal. A properly trained plastic surgeon will be able to guide you through this process. I like to use a cone beam CT scan, ultrasound, and physical exam prior to making any recommendations. I would recommend seeing an American Board of Plastic Surgery certified plastic surgeon, ideally one who also completed an accredited Aesthetic Society fellowship or is a member of the society. At minimum you would need a tip plasty including cephalic trim, lateral crural tensioning/steal, dome/tip suturing, tip graft support, osteotomies, and possibly a high/intermediate/low strip depending on your anatomy and dorsum.

I think it is one of the most beautifying surgeries that we do as plastic surgeons. A large portion of my practice is dedicated to rhinoplasty surgery. It is important to only see plastic surgeons with extensive experience in rhinoplasty, ie perform at least 6 per month and have performed hundreds of cases. Yes, your nose can be improved. However, no physician can guarantee results identical to the photograph you provided. It is an achievable goal. Your limiting factor is the thickness of your skin. You have large cartilage in the tip and that will need to be reduced.
