3 years post septorhinoplasty - what is this lump on my septum? (Photos)

Hello! I have this lump on my columella, it started to develop about a week post op, started small and gradually got bigger to what it is now. I originally thought it was scar tissue however it is hard and painless and almost looks sharp? i Didn’t have any cartilage taken from my ear or rib so I’m wondering if it could be some type of graft poking out? If so, is this a big deal or an easy fix? I’ve been wanting to get it fixed but I’m scared of any complications as I’m super happy with my nose other than this! Thank you

Hello and thank you for your question. Based on your photographs, this is likely scar tissue or a retained suture. I should be relatively easy to remove. Make sure you are going to a surgeon with hundreds of long-term before and after pictures on their website with standardized lighting and standardized backgrounds. With good surgical technique, you could have an outstanding result. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person.

This may be a retained suture or buried skin.  This can be excised under local anesthesia in the office.

Based on the photograph it is likely the protrusion of the medial crural cartilage. It can be easily shaved off under local anesthesia.

I would recommend having your own surgeon examine this directly, since they can see exactly what was done. What you are describing is most likely a swelling or reaction related to one of the sutures placed during your surgery. Right at that corner of the columella, the skin that is cut and lifted while opening the nose in an open rhinoplasty raises up quite easily, which can make the area look like a lump.
