Hello! I measured 16.5mm philtrum length, I want enough change to justify a scar and healing time, but I don't want too much removed where my mouth would look weird? Is 5mm a good middle of the road amount? I trust my doc but I would like a second opinion. I have surgery scheduled in January. (Yes I will dissolve all the lip filler soon, it's not attractive.)

From the pictures I see, and your self described philanturm that is 15mm, I would say that you can be a good candidate for a Lip Lift. This procedure will also help show more incisors when smiling but will also give the upper lip more of a tilt which will help the lip appear more full.
Your request is an interesting one, I believe you have a nicely shaped Cupid's bow that does not necessarily need reshaping, however, it could be that you want more rounding of the red part of the lip and this would bring more balance and create a lip shape similar to the one you have sent me....
