Which brand of smooth round highly cohesive/gummy implants are the best as far as both safety and feeling natural/not too hard? The surgeon I’m going to is double board certified and uses all three brands. Background Info: Female 30’s, Explanted 2 years ago from smooth round Allergan 400 cc implants & want similar size replaced. No complications, no pregnancies, good skin, little breast tissue. I do have a small crease under each breast from Explant I want new implants to fill in
Answer: Implants Hello. I’ve used all 3 brands in the past, and I’ve noticed a few things. Mentir implants feel quite soft, but their rupture rates are higher in most past studies. If you are looking for the “most” cohesive implant, generally that’s going to be allergan’s cohesive. Sientra is the brand I’m using most currently. They also make a cohesive implant (they call it the 107 or HSC+). You likely will benefit from one of these more cohesive implants due to the thinness of your lower breast tissue increasing your risk of rippling. There is no right choice. You chose your surgeon for good reason. Because your surgeon has seen and felt your breast tissue in person, that’s the best person to make a recommendation. Best wishes with your surgery.

Answer: Implants I am not a fan of gummy implants because they are hard, especially in women with little breast tissue. I have used Allergan and Mentor for years and both companies have stood the test of time and both are owned by very large companies that are doing very well. For the past several years, I now use Mentor and they are made in the USA.

Answer: Mini lift with small implants Your breasts are asymmetrical in volume, shape and position. You will need a simultaneous lift. The technique I recommend is a mini lift. Using a circumareola incision, your breast tissue is reshaped, elevated higher on the chest wall and more medial to increase your cleavage. At the same time, small implants can be placed totally submuscular. Best Wishes, Gary Horndeski, M.D.

Answer: Ford, Chrysler or GM If one company was clearly better than the others, then we would all be using the company that makes the better implant. I personally like to have a good relationship with one company and find that by having exclusive relationships with one company I receive better service and this can be very important when it comes to the company backing up implants if surgeons get into a pinch. For example, coming an implant if the surgeon has a problem can be an awesome benefit of quality service from a good company that backs their products. I’ve always judged the companies based on the support I get from their sales reps. I’ve had good experiences with the reps from all three companies. I would go with what your surgeon recommends. It is your surgeons responsibility to guide you through the implant selection process. Some plastic surgeons do this better than others. I don’t think anyone is going to tell you one company is clearly better than the other. There are three key variables that determine quality breast documentation outcomes. The first is the patient candidacy for the procedure. The second is the choice of implants in regards to size, shape, and type. The third variable is the surgeons ability to place the implant in the correct anatomic location. When patients are unhappy with the outcome, the problem is usually related to one of those three variables. The problem is rarely that they used the wrong brand. Focus on the variables you have control over, which is primarily provider selection. In the hands of the right provider implant selection should be pretty straightforward. Best, Mats Hagstrom, MD
