Can I achieve a refined nose tip from a tiplasty using ear cartilage and the nasal septum? (photos)

Based on your photos, you are a great candidate for rhinoplasty. Septal cartilage can be used to strengthen and further define the tip. It might help the width of the nose to bring in the nasal bones a little bit though. Be sure to see an experienced rhinoplasty surgeon who can do computer imaging for you to see what you might like the most.

Ear cartilage is too weak to be utilized in rhinoplasty, in my professional opinion. If you have not had any previous septal surgeries, there should be sufficient septum for tip grafting. You are a good candidate for a rhinoplasty procedure. I believe that your goals are obtainable. I recommend a real plastic surgeon with 6-8 years of plastic surgery training. There are many people performing rhinoplasties who are not actually real plastic surgeons.

You may be able to achieve what you're looking for with ear and septal cartilage, but it may depend on the quality of your septal cartilage. It's very common for Asian patients to have very small and thin septal cartilage that may not be reliable enough for the desired result.

I would discuss with your surgeon the possibility of needing additional cartilage for support such as rib cartilage. You may have options for utilizing either autologous or donor rib cartilage.

the answer to this is dependent on what your goals for final aesthetic are. If you are not wanting/do not need dorsal augmentation, less cartilage is required. However, in many asian patients there is a relative lack of available cartilage from the septum. Depending on the need for changes to tip position and projection, rib cartilage may be preferred to ensure adequate strength, long-term to maintain your results. In many asian rhinoplasties, I discuss rib graft with my patients prior to surgery. This way, if the decision is made intra-operatively, that this will give the best results I am able to use it. A major concern with using ear cartilage or weak septum is that it may not provide the support needed, long-term. Tip refinement, narrowing of the nasal width are easily addressed when performed by a well qualified rhinoplasty surgeon.
