I had a DP facelift and N/L in Jan. I went in for these puffs on the sides of my mouth area and my sagging neck and was told no problem to correct these. In February the puffs started returning and the neck has issues as well still. I’d like feedback and thoughts on if indeed I was given a deep plane and neck lift? What needs to be done to correct these issues? Thank you
Answer: Deep Plane Facelift and Necklift It's impossible to give commentary on how your procedure was performed. However, the photos show evidence of platysmal bands, which should not be present if a properly performed deep plane facelift/ necklift was performed 3 months ago. A review of your operative notes and most likely a revision will be necessary.

Answer: Facelift Recurrence of the fold to the side of the mouth and platysmal bands can both recur after any type of facelift, so it is impossible to tell whether or not you had an actual deep plane facelift or another variety. You may require a revision surgery to further correct these issues, and your surgeon at the time of the revision may be able to tell you at that point.

You still have some fullness in the submandibular area that is likely due to the submandibular glands and medial platysmal banding. While you may have had a deep plane facelift, you've had too conservative of an operation in the neck. Deep neck surgery involving pulling the platysma muscles together and possibly reducing the size of the submandibular glands (versus additional tightening of the platysma muscle behind the jawline) would improve the concerns. In short, further surgery is required to improve these concerns.

Answer: Correct I would recommend a revision facelift/necklift. The platysmal bands are still visible in the midline. There is still some laxity and prominence in the jowl area. I would highly recommend finding a board-certified or board-eligible plastic surgeon with 6 years of integrated plastic surgery residency training and aesthetic fellowship training. This highly differs from all other specialties who claim to be plastic surgeons including "facial plastic surgeon", "oculoplastic surgeon", "cosmetic surgeons", and other imposters out there.
