Would a second round BBL help me achieve the roundness that I am looking for? (Photos)

Based on your pictures you you appear to be good candidate for full tummy tuck and intense liposuction also, I suggested take the fat we get and transfer to the buttocks and hips, to mold them and get better volume, contour and butt projection, is that we know as "Brazilian Butt Lift" (BBL).

You are in good company with many others who had high hopes for BBL but instead received disappointing results (lumpiness, unevenness, lack of projection in the central and/or lower buttock, post-deflation sagging, etc.) as most of the fat melts away. It is not quite as much the quality of the doctor nor liposuction technique but rather the quality of the procedure that is a large part of the problem. Thus, going back for round 2 BBL most often produces the same disappointing results but at another financial loss. Buttock implants indeed provide a more reliable, predictable, relatively safe, and permanent result, especially for adding projection to the buttock and often smoothing out lumpiness. Occasionally a lower buttock tuck may be advantageous for lower buttock fold asymmetries and/or sagging. Please keep in mind that an extremely few number of board-certified plastic surgeons specialize in buttock implants so be sure to consult with one that is reputable and experienced. Unfortunately you may have to travel long and far away from home to find this particular surgeon. Glad to help.

I understand your concern. However, without a proper assessment, it would be difficult to determine what went wrong. It is best that you visit your plastic surgeon for further assessment or ask for a second opinion. Only after a thorough examination, you can get proper recommendations and advice.

Doing a round 2 lipo/BBL has a 50% chance of giving you the desired "roundness" . In Florida the new required ultrasound guided BBL adds to the fees. I now charge $7,900. Another option would be a combination surgery of full TT with 1 liter lipo (as allowed in Florida doing combination TT surgery with liposuction in office surgery setting). Fee $10,000. What you need is a BBL revision utilizing a lateral thigh and buttock tuck. This will lift your buttock up and round you out at the same time. Please also consider extending the incision to the front so you can tighten up your tummy so that you achieve a high def body contouring results. When making an assessment on the outcome of a procedure patient need to always include proper before and after pictures. If you don’t have before and after pictures then ask the surgeon who did your procedure to forward the ones they took.
Based on your current pictures I don’t think you were a good candidate for a BBL in the first place.
Your bottom is certainly not lacking volume and fat transfer primarily is a volume adding procedure.
The flat spot on the lower part of your buttocks is due to connective tissue tethering the skin in that area will not project by adding more fat while the surrounding area as well. The shape is predictable and an ethical talented an experienced surgeon should have been able to tell you exactly what the results would look like and steer you in the right direction.
Adding more fat will make it worse so you’re not a good candidate for a second BBL.
Considering you probably weren’t a good candidate to start with revision work becomes more difficult than primary work.
Getting the bottom do you want may be difficult to achieve.
Recontouring the transition of your buttocks to the lower back and transition of your buttocks to your love handles and reducing the upper part of your bottom is probably the best that can be achieved.
Delivering consistent quality liposuction and fat transfer results is more difficult than most people believe it. It is imperative patients interview multiple plastic surgeons carefully to find the most talented and experienced providers. finding providers for also honest, ethical and service patient advocates is an added bonus.
Doing that requires a substantial amount of work by having multiple in person consultations.
During consultations ask each provider to show their entire collection of before and after pictures especially of patients who have similar body characteristics to your own.
Had your surgeon done that or had you requested that then you would know what the results would look like.
In the end your surgeon is responsible for the outcome of the procedure and you are responsible for choosing that surgeon.
If you want a proper second opinion consultation then they should be done as in person consultations and you should come prepared with a copy of your operative report and all before and after pictures which are available from your first surgeons office if requested.
