I got my breast implants several years ago. They are about 300cc’s over/ under the muscle saline. I have had children and think my breasts look good and aren’t that saggy. They always looked very natural. I am going to have a MM and wondering if I need a lift or I can just get a larger implant? I realize if one day I may need a lift down the road but wondering if I can get away with just larger maybe HP implant this time. I would like a bigger cup size. Thank you.
Answer: Breast surgery I don’t see anything specifically wrong with the breasts. Saline implants don’t need to be switched unless there is a problem despite the rumor that they need to be replaced every ten years. They do not. Your excellent result is testament to the durability of saline implants as well as having fewer problems than silicone.

Answer: Revision You don't "NEED" anything with your breasts. That being said, your saline implants that have served you well are now 20 years old and at some point, you will likely have a deflation. Since you are having surgery anyway, this is a perfect time to "fine-tune" your breasts. Over the last 20 years, your implants have placed weight on your soft tissues, whihc have, in turn, stretched a bit. You could easily have a simple pocket tightening procedure called "popcorning" which uses thermal energy to shrink your capsuls/pockets. Using a slightly higher profile/slightly larger implant will give you the look you are trying to achieve without being an overly invasive or difficult procedure. Additionally, I would also recommend circumareolar mastopexies for proper nipple areola complex repositiopning and improved symmetry, but that would be optional and based on what it looks like with your new implants in place. Did you discuss your question with your PS who is going to do the MM? I cannot tell if your skin is loose or not. If so, a lift may help tighten loose skin around a larger implant. Otherwise, your shape looks nice and a larger implant may be all the you need. I also agree with the doc who said that over time, your breasts will sag more with the increased weight of a larger implant.Sorry , not to give you a definitive answer, but it would be best to get an in person exam.Best Wishes,Dr. Christine Rodgers

Generally speaking, if the nipples are lower than the inframammary line the patient will most likely need a breast lift. If the nipples are at or above the inframammary line we can generally get by with just a breast augmentation depending on the size of the implant the patient wants. I often still using implant when I perform a breast lift because it provides more upper breast fullness after the lift. To be sure a consultation with a board-certified plastic surgeon will help determine what the patient actually needs.
