I got a breast augmentation in Miami last year. Dr put 560ccs smooth round silicone implants in both breast. I’ve always had an issue with a wide space in between my breast and I brought that up to him and what he told me was that I would always have a space but they would just be bigger. I sometimes have pain from the implant going to far into my armpit. Can this be fixed?

Answer: Breast Revision Thank you for your question. A large portion of my practice is dedicated to revisionary breast surgery, and I fix problems very similar to yours very commonly. You have lateral and inferior malposition of your implants. I would perfom an inferior and lateral capsulorrhaphy (suturing of the pocket) and mirror image capsulotomy (cutting of the pocket) to bring your implants higher up and closer together. I wrote the first paper on capsulorrhaphy. Unfortunately, your areolas point down and out, and this would worsen this. You definitely would need a lollipop lift to reposition your areolas higher and more to the center. You would be able to have an excellent result, but not without the lift. Best wishes to you.

Moving the implants too far medially can result in synmastia or unaboob which is very difficult to correct. That being said, the right amount of pocket dissection and implant selection can create excellent cleavage. Breast revision surgery is the most difficult plastic surgery procedure performed. Botched breast surgery alters normal tissue planes and laxity, the ability of breasts to heal properly, and leaves scar tissue that dramatically affects the level of difficulty of the corrective surgery.Many patients come in to correct poor outcomes from their initial surgeries. The most common problems are due to implant malposition, bottoming out of the implant, poor scarring, and incorrect implant size.While the type of revision required will depend on the result of the previous surgery and the desired result of the patient, some revisions involve extensive pocket repair that may or may not need specialized external bras.If you are considering breast surgery revision, you should speak with a board certified plastic surgeon and have yourself properly assessed.
