The photos show very large, heavy, ptotic breasts with the nipple and lower breast sitting low on the chest. If the breasts are truly getting larger as well as hanging more, the first step is to look for the reason: weight change, hormonal changes, natural breast tissue growth, skin quality, and stretching after a prior lift can all contribute. If growth is rapid, new, painful, or one-sided, a breast exam and appropriate imaging should be done before planning cosmetic surgery. For a more durable result, a revision breast lift alone may not be enough if the breast tissue is heavy. A breast reduction with lift, removing enough lower-pole and side breast tissue to reduce weight, often gives better support and may slow recurrent sagging. The exact scar pattern, amount of tissue removal, and whether any internal support is useful would need an in-person exam, measurements, and review of your prior operative details. Stable weight and a clear goal for final size are important before revising.
