4 months post op Voluma in cheeks, now having pain. Do I need to be worried?

What you are describing is not a typical reaction. Initial soreness after injection is normal, but pain returning several months after injection is not. I encourage you to return to your injector for an evaluation.

While it is unusual to have pain this far out from the filler, I have seen delayed reactions to the filler such as delayed onset allergy and or infection. Sometimes it's hard to differentiate the two. If there is an allergic, infectious, or some other response to the filler you will typically have swelling, redness and/or itching in addition to the pain.  I suggest seeing you injector to get a thorough exam. If it is the filler it can be dissolved with hyaluronidase, although volume is more difficult than some other hyaluronic acid fillers to dissolve due to its cross linking and way it is made.
