Always have had a hard time breathing through my nose resulting in me being a chronic mouth breather which I believe has contributed to how my face looks as an adult. I want surgery to give myself a more 'defined' face and fix the asymmetry problems that I face.

For patients with long faces, a few quick fixes can be an anatomical chin implant widen the appearance of the jaw and give more anterior projection.  Additionally dermal fillers such as Vollure, Voluma or Radiesse on your cheeks and jawline can help add definition and shape.

Thanks for your question. There are are variety of options when it comes to improving facial symmetry. These include fillers, either temporary or permanent, facial implants and fat grafting. They can create volume, symmetry, and balance your facial features. A variety of options is available to treat both functional (breathing, bite) and aesthetic problems related to a long face. Orthoghnatic surgery with lower and upper jaw manipulation, septo-rhinoplasty, sliding genioplasty, are procedures that will address both problems if executed properly. However, camouflaging procedures with chin and cheek augmentation with implants, possibly jaw augmentation as well as nose surgery are procedures that  will  address mostly aesthetic aspect of your problem. A consultation with at least 2  surgeons (plastic and maxillofacial) should be your next consideration. Good luck.

Symmetry and contouring of the face can be improved with well-placed fillers (Voluma, Radiesse, Bellafill) and Sculptra. Fat or implants are alternatives. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer

Hello and thank you for your question. Based on your
photograph, you may benefit from a tip refining rhinoplasty and dorsal hump reduction.  A chin implant can be performed at the same time for
improved jaw line contour.  I always use a medpor implant with screw
fixation, which stays in place over time.  A silicone chin implant has a
high rate of malrotation over time.  The overall result can make your face appear less long.  Make
sure you specifically look at before and after pictures of real patients
who have had this surgery performed by your surgeon and not just a computer
animation system. The most important aspect is to find a
surgeon you are comfortable with. I recommend that you seek consultation with a
qualified board-certified plastic surgeon who can evaluate you in person.

Both implants and fillers in the cheeks and chin can create good results for patients.  Both can build up the cheeks or bring symmetry to the face.  You must balance the pros and cons of each procedure and do what you feel is the best decision for you.
Fillers:
Pros: No downtime Reversible Quick
Cons: Temporary More expensive in long run
Implants Pros: Permanent Great for patients with poor bone structure Less expensive overtime
Negative: Risks associated with surgery: Possible infection, Downtime

With a long narrow face and a short chin/lower jaw, you would need procedures that provide forward projection and facial widening. The combination of chin and jawline augmentation with male cheek implants will change the shape of your face into an apparent shorter and wider one. Given the need for multiple dimensions of facial changes and existing asymmetries, such augmentations are best done with 3D CT planning and custom implant designs.

The  side profile photograph demonstrates an overly projecting nose with the dorsal hump, and an under projecting chin. While a septoplasty will improve airflow dynamics through the nose, a cosmetic  rhinoplasty can accomplish decreasing the overall projection of the nose and shaving down the dorsal hump. Placement of a chin implant will augment the chin forward to help with facial balance and proportions, especially from the side profile with respect to the nose. Cheek implants are only placed for patients who have a flat mid face  and a flat maxilla. Cheek augmentation can be done with fillers such as Voluma, Sculptra, Bellafill and even your own fat.  The duration of those products vary but can last multiple years depending on which you choose.  The other option is a cheek implant.  To give a stronger, defined look to your face I would also recommend a chin implant.  
Seek out a board certified facial plastic surgeon in your area for a formal consultation to discuss your options.  Often times imaging software can be used to give you a virtual sneak peek at what changes can be made.  Best of luck!
