Am I a good candidate for a neck/facelift @ almost 36?

Question

Turning 36 soon and have lost over 105lbs. I have always had a full Turkey neck. I had lipo only back in 2016/2017 and had no skin retraction. The lipo, weight loss & genetics left me with an extreme amount of lax, excess skin on my neck. I've been told an SMAS/MACS mix with temple to behind the ear incisions and possibly down the neck hair line. He will do some extra lipo & muscle tightening. I will be awake under twilight in office if I go with him. Im just terrified of laxity coming back.

Very likely, based onthese photos and short of in person evaluation, that anterior neck lift may be good option for you. Howerver. only on the basis of in person evaluation one could decide whether neck lift of lower fce/neck lift would be better solution for you. Good luck.

Congratulations on your weight loss.  I would not do a MACS lift on you. It will not address your biggest issue, the excess skin and muscle laxity in your neck.   You need a formal platysmaplasty and neck lift.  If your jowls are also bothering you then you also need a lower facelift.  I know that providers are doing these under local anesthesia in the office but I think that it is not a positive experience, no matter how much valium they give you without some sort of IV sedation.

Based on the significant weight loss (congratulations) and photos showing skin excess, the best procedure available in this circumstance would be a deep plane facelift.  This obviously includes a deep neck lift and neck contouring.  Liposuction is routinely performed during a facelift, but most of the fat in the neck is deeper and not treatable with liposuction.

Hello! Great question and thanks for the photos. 36 years old is a bit young to consider a face and necklift given that even the best techniques don't last forever. There is upkeep associated with any procedure of the face and neck long-term. This doesn't mean that the laxity would come back to the same degree. In your case, you may very well benefit from the procedure and may have great results. It's just important to remember your age and future maintenance to keep your results looking optimal. From your photographs and your Weight loss history, the only way to tighten that loose and inelastic skin in the neck is by performing lower face and neck lift procedure. The reason liposuction may not have been very effective is because you probably have fat deposits located underneath the platysma muscle which would have to be surgically removed with a face and neck lift. Choose your facelift surgeon wisely based upon extensive experience producing natural results, since you're very young for this procedure.
