for reference I had a full neck lift w lipo and platysmal plasty I have maintained a consistent weight since post op and followed aftercare instructions to a T. my surgeon claims my results are optimal but I am deeply unsatisfied with my results. Did my neck lift fail?

I would suggest facial liposuction of the cheek and neck region to be combined with that same time or several months later by a facelift and a necklift. The liposuction needs to be aggressive to remove excess fat. Commonly liposuction is not performed with a facelift or neck lift. I believe that the problem you are faced with is that your volume excess under the chin is situated deep to the platysma muscles and can be addressed with deep plane neck lift surgery rather than with typical neck lift surgery. The structures that are generally reduced in order to operate such changes include some fatty tissue deep to the platysma, the fullness of the digastric muscles and the lower part of the submandibular glands. The characteristics of these elements is often influenced by heredity and is not significantly influenced by weight. However, the deep plane neck lift can improve on these features.

Answer: More lipo You may need more liposuction. Sometimes if there is still some fat, it may not show right after surgery because the skin is pulled tightly but once the skin tension settles, the fat shows itself.
