I had BA in 1989 and have never had any issues. Because of their age, I had an MRI which showed no intra or extra capsular rupture. Is that unusual and should I think about replacement even though everything looks normal?

You did not post photos, so I would suggest meeting with a board certified plastic surgeon and discussing your concerns and options. Implants can be placed in front of the muscle and fat grafting can be added for more tissue for coverage of the implants if needed.
