Unfortunately, you have not provided your before photographs. At this time, your breasts are too low on the chest wall and too widely separated. The technique I recommend is explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Your body habitus is extremely lean, making this issue of looking augmented difficult to correct. It appears that your implants are over the muscle; if I am correct, placing new implants under the muscle will be very helpful at masking them with your muscle in critical areas at the midline and upper poles. The next is to change to highly cohesive silicone gel implants as they will have a softer shape and feel. More important than profile or size, implant diameter should be reconsidered along with pocket position. Your issues are complex so your surgeon needs to be facile at revision surgery and the use of resorbable mesh.

Thank you for your question. A large portion my practice is dedicated to breast revision surgery, and I have been in practice for 30 years. Most likely, your implants are on top of the muscle and that is what gives you the "fake "look and why your breasts are so spread apart. If you go with a lower profile implant, you will just have more wrinkling. Your mastopexy-lift was poorly performed and would need to be revised. My recommendation would be to place the implants under the muscle and redo the lift. You should be able to have an beautiful result. Best wishes to you.

If your implants are behind the muscle, there is some scar tissue pulling on the muscle when you raise your arms. A lower profile implant will be wider and will help decrease the gap. Naturally, your breasts sit apart, and they cannot be moved to be closer. A wider based, lower profile implant will help to improve this.

Based on your nicely post of photos, I would do a revision of the pocket with more dissection toward the breast bone and use a slightly larger implant, medium or moderate profile to a 300 to 450 cc implant using sizers. For the dent in the vertical incision, I would use micro and macro crafting. Fee from $8000-$10,000. Best of virtual consulate.

To start with we always need to see proper before and after pictures to make an assessment regarding any plastic surgery outcome.
Breast augmentation outcomes are usually based on three variables.
The first is patient can see. The second is implant selection and the third is the surgeons ability to put the implant in the correct anatomic location.
And your case your candidacy for breast documentation is by far the biggest issue.
Even though we don’t have pictures, there are some candidacy variables that stand out as being significant.
First and foremost, your breast sit very wide on your chest wall. Secondly, you have significant breast divergence. These variables are innocent put on display or amplified with the projection of breast implants.
Patience that have ideal breasts that point forward we’re going to have those ideal breasts, be amplified, and look great after implants.
if the breast don’t look so great then that is going to be amplified with the implants.
The surgeon can’t move it where your breast sits on the chest wall.
Likewise, the surgeon can’t change the divergent angle of your breast.
For patient who have breast that sit very wide on the chest wall, using wide implants or lower profile implants May be an improvement in someways and cost more problems in other ways. (High profile implants have a smaller diameter and more projection while low profile implants have less projection and a wider diameter)
A wider implant may reach closer to the midline, but will also reach further towards your armpit.
I wonder if your implants were placed under the pectoralis muscle.
Doing so it would probably be better in your case.
I think the most important issue for you is to have a very honest consultation where all of your candidacy variables are brought up so you have a clear understanding of what the breast augmentation is going to look like.
Your surgeon didn’t really do anything wrong.
Maybe something could’ve been done slightly differently, but the overall look or outcome of your procedure is almost entirely based on your candidacy for breast Augmentation.I say that without even haven’t seen your before pictures.
Based on what I see, your best option would most likely to have gone under the pectoralis muscle using small implants that were low profile.
It looks like you had a breast lift as well.
Moving the nipples towards the midline may seem tempting, but the nipple also needs to be centered on the breast.
Overall I think your outcome is fairly straightforward.
Switching to a different shape implant may have some impact but overall the outcome is based on your candidacy for the procedure.
Ideally, this should be discussed in person consultations.
