I have TBD & my breasts are super saggy & far apart, 1.5 in at the top. I know it's determined by my anatomy but is there a way for me to tell how close together they'll be after a lift & augmentation? Can I tell where my new nipple placement will be? What will determine their new shape? (I dont want square/boxy breasts). I would like them to be rounded with nipples that point forward not outward, is that possible? I struggle to find B&A photos w/ patients similar to me

Your anatomical problem is at an extreme. Routine surgical planning won't work.
Your nipples need to move inward and will require a full "lift" but modified to accomplish.

I would recommend bilateral breast augmentation with submuscular dual plane pocket. I would see a surgeon with the ability to show you the potential results with 3D preoperative imaging (Vectra or Crisalix or Photoshop).
