I think that you are on the right track with the lift portion of your request. Removing some skin with the lift will tighten your skin envelope and hopefully create some additional cleavage. You will not require much of a lift in terms of nipple position but resection of some of the lower pole skin can give you the more youthful breast appearance that you desire. I would make sure that you are consulting with board certified plastic surgeons to ensure the highest level of care.

Naturally, your breasts sit low on your chest and the bases cannot be moved. Your implants are well positioned now based on your anatomy. Smaller implants would look less "matronly". You can not have "perky" or high breasts.

For what you are describing we usually still would use a small implant and do a full mastopexy to create the perky breast you desire. Depending on the size you want, sometimes we remove breast tissue from the bottom because post operatively breast tissue is the first to droop overtime with gravity.
