I think that you are on the right track with the lift portion of your request. Removing some skin with the lift will tighten your skin envelope and hopefully create some additional cleavage. You will not require much of a lift in terms of nipple position but resection of some of the lower pole skin can give you the more youthful breast appearance that you desire. I would make sure that you are consulting with board certified plastic surgeons to ensure the highest level of care.

Hello, thank you for your question and sharing your photos. It really depends on what your expectations are and your wish pics. For what you are describing we usually still would use a small implant and do a full mastopexy to create the perky breast you desire. Depending on the size you want, sometimes we remove breast tissue from the bottom because post operatively breast tissue is the first to droop overtime with gravity. At this time, I recommend explantation and lift using The Bellesoma Method. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

I find it incredulous that a surgeon would take you to the operating room and not perform an agreed upon procedure, i.e. a formal lift versus what you received, a crescent lift (which is a worthless endeavor in deforming your areola and not lifting the lower pole of your breast one millimeter). As for the three wise men that told you that you don't have ptosis is amazing. You had ptosis, and now you have more ptosis. You need to find a breast expert; using docs like this will not get you to your goal and put you at risk for new problems.

I do think a smaller implant combined with a lift would provide a fresher look for you. You could qualify for a narrower implant with the pexy. You do have a "lower footprint" breast so you are limited in how high your cleavage will go.

You will be a great candidate for an anchor breast lift/reduction in order to raised the inframammary fold about 4-5 cm, and move the footprint higher on your chest and achieve a more youthful appearance of your breast in relationship with your chest.
Definitely the change for smaller implants will be needed.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

The crescent lift did not provide the lift you needed. It can be converted to a full lift but there are significant scars involved

Based on your pre-op photos, you should have been given an option of better breast lift than crescent, most likely vertical, lollipop. However, that can be done now; have in mind that your breasts are sitting low on your chest wall and moderate nipple/areola migration is needed. It will also tighten your lower pole and give you smaller, perkier breast, possibly better cleavage. Good luck.

Hello @efmolly, thank you for your question. You could consider a breast lift. This procedure will remove excess skin, reposition breast tissue in order to create a better shape and cleavage. The scar can be varied, mostly the inverted T- shaped is used however in my experience of 22 years I have made a modification to improve the extension of the scar being the L shape for minimal scarring and the most adequate result.
