Revision - Should I get more of a tip lift? (photo)

Question

I'm going to have a revision. I don't like how my tip stands, i think it is too much of a V and on 3/4 views my tip looks weird for my face. Is it because it is not lifted enough? If I get more of a lift, would my nostrils show (I don't want that!)? Or do I have a restricted columellar and that is causing the droopy look... Help!

It is possible to do revision on the nasal tip only.  The nasal tip has a 
number of sub-units that can impart a certain look at the nose.  Proper analysis 
of nasal tip anatomy can is important to guide one in proper surgical maneuvers 
to achieve esthetic improvement.  It appears from your limited photos that the 
infra-lobule component of the tip is a bit droopy.  Careful surgical correction 
can give you more of a lifted look without having your nostrils show.

It is fairly straightforward to lift the tip during revision rhinoplasty. Minor lifting can be done as a short office procedure under local anesthesia with an incision inside the nose.  It is difficult to tell what is the best way to aesthetically improve your nose from the supplied pictures, but I would agree that some lifting of the tip would likely be beneficial. There are several techniques to lift the tip depending on the amount of tip support, the length of the septum, and the soft tissues inside the nose, and this is a common reason for revision. It is much better to have a nose that droops slightly than one that is too short.  A short nose requires much more surgery to correct, and gives the nose a veryoperated on appearance.

Revision rhinoplasty is commonly performed to lift the tip of a nose that has either fallen after a primary rhinoplasty or was not lifted enough. There are different techniques used to lift the tip. In order to fully evaluate your candidacy, more complete photos would be necessary as well as examination of the nose and septum. In the vast majority of situations the nose CAN be lifted and in such a way where the nostrils will not show more.

From your photos, it is a little difficult to fully assess your facial proportions. It does, however, appear that lifting your tip further would be a good idea. You will, however, likely see at least a little more nostril on front view when doing so.

The tip can be lifted and reshaped depending upon your  goals for the procedure. Lifting the tip slightly 2-3 mm result  can be performed without giving any nostril show.
