If you wish only to correct for the nipple size and descent then a breast lift would serve you well to correct for this plus give you an opportunity to have improved breast symmetry with respect to shape and size. If your mandate is also to reduce your breast size than a breast reduction would be more appropriate.

I completely agree with Dr Blinski. Looking at your picture, independently of the sutures that may be used, I don't think you would be a candidate for areolar reduction.
