There are many factors that can create this type of asymmetry after augmentation. First and frequently neglected is a small asymmetry present before surgery which was not brought to patient attention before surgery. Weight change, uneven position of breast folds, breast implant contracture, uneven placement of implants, etc.. If one looks at your before surgery photo, some asymmetry seem to be there? It is best to discuss these issues with your surgeon and seek appropriate correction. Hope this helps.

Thank you for your question and posting appropriate pictures. You may be a candidate for breast implant revision, which occasionally can be performed under local anesthesia. It is best to have a virtual consultation to ascertain muscle activity. While you did have some asymmetry preoperatively, there is room for improvement without an actual lift.

Although you preop photo is slatting, you can see that your left breast sits lower than your right. This is not corrected with just an augmentation, so it still sits lower. the left implant also may still be sitting a little high. A small lift should be able to improve things.

Based on the nicely posted series of photos appears the left breast is smaller and slightly ptotic. I recommend exchange left implant to 550 cc HP with right donut lift. Fee $4,000+ Best to virtual consult with..

You had significant asymmetries of your breasts pre-op. If the breast with the nipple pointing downward also appears to be the higher breast, then revising it should lower the bottom of the breast and rotate the nipple higher. A periareolar mastopexy (scar around the nipple to raise the height) may be an option as well.
It may be that the left breast lacks lower breast tissue allowing for better symmetry.
