I’m a 34 year old male who needs to add support, structure and volume to his midface which I want to use to inturn provide me under eye support. I find that adding filler directly in the undereye area provides me volume but not support. I also find that filler provides support in the mid face temporarily but once it becomes “soft” after a few months, it just provides volume. Does Sculptra have a lifting and support effect and does it tighten the SMAS layer and not just volume? Also, Do threads provide structure and support to the midface and not just volume? I want to use threads as a trial for a midface lift.
Answer: Facial Contouring with Fillers (Juvederm Voluma, Vollure, Volbella, Radiesse, Bellafil), Sculptra, Fat Grafting, Implants Facial sculpting and contouring can be created with well-placed fillers (Voluma, Countour, Radiesse, Bellafill) and Sculptra, which would give improvement in facial balance. Fat or implants are alternatives. I recommend getting a formal evaluation with a facial sculpting expert. Best, Dr. Emer

Answer: Scuptra is a Wonderful Volumizer. Stay Away From Threads Sculptra is an injectable form of L Polylactic Acid, which stimulates collagen production in the face, incrementally adding volume over a 4-6 week period. The plane of injection is deep, lifting all facial structures. Usually 2-4 treatment sessions are required to reach the endpoint. On the other hand, thread lifts are an incredibly expensive, short lived result type procedure that make no anatomic sense. Translation: waste of money.

You are correct in that when treating the under-eyes, putting filler in tear troughs only is not a comprehensive solution. Usually treating under-eyes involves adding cheek and mid-face structure. Mid-face filler, when done in the right amounts and with high quality products, should last about 12-18 months. If it only lasts "a few months" it may because a cheaper product was used, or not enough of it was used. Sculptra is a collagen stimulator. It can firm up the skin, improve skin quality, and help with volume. It is a subtle and natural result. It's lifting and support effect is more limited compared to filler in my opinion. I counsel patients that we put the scaffolding (structure) in with filler, and then we improve the overlying tissues by boosting collagen with Scultpra. Threads are an interesting new modality however the threads that are available in Canada currently do not last very long. Many patients find that their results with threads are lasting 6 months or less.

Threads also provide no support, except for some suspension of the mid face, but that suspension support will disappear with a first rather hard face washing. I have never seen a thread lifts hold up even three months at best. if you desire to have good support as well as volume in the mid face, you can use Sculptra, however, you cannot come close to the eyelid or a process of granuloma formation may occur. My best recommendation to you is to have a plastic surgeon who has experience in fat grafting techniques , perform fat grafting to whatever parts of your face requires it, and this will intern, restore the lost volume and provide substantial support. When the procedure of fat grafting is performed by an experienced plastic surgeon, the results are extremely natural and practically permanent. However soft tissue fillers do not structurally lift and support tissues rather they are volume enhancers. Thread lifts provide only very very temporary lifting of facial skin and their potential complications which can be disfiguring. Most patients in my experience who have had thread lifts have been disappointed in the duration of the improvement.
