Because you do not have a problem of skin laxity, it is possible to do an anterior necklift and reduce the volume both above and under the platysma which involves fatty tissue, reduction in the volume of the digastric muscles and potentially reduction of the lower part of the submandibular glands. The same access under the chin can be used to insert a chin implant if you also wish to increase the chin projection.

This is my expertise and a huge portion of my practice. Asymmetry is normal and expected. Buccal fat pad gives slimming not symmetry. Masseter botox will make you slimmer and make the jawline more loose. Fillers help give shaping and contouring. Looking at the photos you have provided I would recommend some liposuction of the neck and yes you could do some fat transfer to the face to add some cheek volume and soften the NLF area. Based on these photos, you are a candidate for chin augmentation with implant. Whether agressive neck liposuction or neck lift with liposuction would be a better solution in your case will depend on in person evaluation and discussion. Good luck.

"Double chin” is one of the most frustrating issues for patients, because it can be a stubborn area that does not respond to exercise or diet. Until recently, the only solution was surgery. KYBELLA® (deoxycholic acid) injection is the first and only FDA-approved injectable treatment that is used in adults to improve the appearance and profile of moderate to severe fat below the chin (submental fat), also called, “double chin.” Kybella is administered via injection, and two to four treatments are recommended over an 8-12 week period. It is effective for even a small amount of fat under the chin.
You may also get improved neck contour with small incision surgery using a combination of liposculpture techniques that remove fatty neck deposits and help tighten skin. The neck lift is an easy way to reduce jowls, eliminate a double chin, slim the bands of muscle that form the wattle, and tighten neck skin. Typically, a surgeon will also decide to perform a lipectomy, suctioning out fat in order to resculpt the area. A lipectomy is performed through an incision less than a centimeter long under the chin and a 2 mm incision in the crease of the earlobe. A cannula is then inserted and fat is liposuctioned out in order to resculpt the neckline. If a neck lift is also being performed, excess skin will be removed. Par ayudarte con tu rostro, la mejor opción para ti es una liposuccion de papada, aumento de menton con un implante y quitar las bolsas de bichat. Si te ponen grasa, existe la posibilidad de que se absorba por tu cuerpo. Asi mismo, podemos poner un filler para darle más volumen a los pómulos y a la mandibular pero eso ya es en una segunda instancia. Puedes empezar por el implante de menton y ver tu resultado, si te sientes cómoda con eso así quedate.
