After blepharoplasty, your lower eyelids are vertically and horizontally short. You have lid margin rotation with outward rotation of the lower eyelid water line. A physical examination is likely to show that scar tissue limits the upward excursion of the lower eyelid. Please understand that lateral canthal surgery or any of the compromise lower eyelid procedures used to address this issue, lack the ability to relive your concerns. I would advise you to be especially weary of well meaning surgeons who propose "extra-strong" lateral canthoplasty using a drill hole or other lateral canthal tightening procedures. You are short of skin and eyelid soft tissue. Do not get talked into an ugly skin graft. I employ a method where I vertically recruit skin and cheek soft tissue into the lower eyelid space and permanent hold this in place with the use of a rim implant hand carved from ePTFE and fixed to the bone with micro screws. This approach permanently holds this tissue to augment and restore the vertical height of the lower eyelid. A hard palate graft from the roof of the mouth is also used to control the shape of the lower eyelid. This allows the lateral canthal angle to be repaired under no tension. This type of work is not available were you live. I am attaching a video of how this surgery is performed.
