Lateral canthal surgery alone will not repair your concerns. Many of the procedure offered by my colleagues to address these are compromise procedures. Releasing the lower eyelid retractors and trying to make a stronger lateral canthoplasty will not fix this. My approach to this considers the shape of the orbital rim that supports this area. I also recruit skin to lengthen the lower eyelid by vertically lifting the cheek soft tissue vertically. Also a hard palate graft is utilized to shape the lower eyelid. These measures allow the canthoplasty to be performed without tension. There is no substitute for a detailed in person assessment. Your eyes can be your best feature.
