though rare, some women who undergo breast augmentation will develop a minor deformity known as a “double bubble,” in which additional folds appear underneath the breasts due to the implant accidentally shifting or contracting post-surgery. In most cases, the implant simply slipped too far down the chest wall and traveled behind the breast inframammary crease (the area where the lower breast meets the chest), instead of moving forward to fill the breast cavity. If left untreated, the lopsided implant will form unnatural indentations, resulting in an odd “four-breast effect” rather than the full, rounded chest the patient initially desired.

Double bubble is a catch all term that essentially describes problems with the inframammary fold. You essentially have a bilateral double bubble deformity characterized by inadequately obliterated original (native) breast fold that has now transposed on to the lower pole of the breast. The new breast fold is where the implant settled down to. This is how one has a "double bubble". This is a complex challenging case due to the need to obliterate the original breast fold while not destabilizing the remaining pocket/new fold position. You truly need to see a breast revision expert certified by the American Board of Plastic Surgery. Your revisionary surgeon will likely include extensive breast pocket work,, placement of new implants (larger and more high profile in my opinion), use of internal support (i.e. Galaflex), and drain placement.

You’re posted photos to show significant double bubble deformity, and the option of self fat grafting, maneuver, FD approved, fat grafting or the new ALLOCLAE FDA approved fat grafting ARE OPTIONS, BUT THE COST FOR BOTH THE FDA APPROVED INJECTIONS ARE VERY HIGH VERSUS DOING LIPOSUCTION AND YOUR FAT GRAFTING. I don’t think volume enhancement is going to fix your problem.
The outcome is the director reflection of how the procedure was done.
At this point, it may be challenging to get a good outcome.
I recommend you focus on provider selection, making sure you’re working with as efficiently skilled plastic surgeon.
When consulting with plastic surgeons for revision procedures, ask them to show you examples of previous patients who had similar situations for whatever procedure the surgeon is recommending.
Finding the right surgeon to work with can be really difficult.
I recommend having several in person consultations before selecting a provider to work with.
