13 year old 600cc implant exchange! Desiring to go much smaller - Is it possible to go smaller than 450cc?

Question

I’ve had 600cc smooth, high-profile implants under the muscle for 13 years. Due to a rupture, I’m planning a revision. One surgeon recommended going down to 450 or 480cc Mentor Boost implants (moderate profile), placed dual plane. They said my chest is petite, my current implants are too wide, and suggested a width around 13–13.5 cm. I’m 5’6”, 145lbs, athletic build, and very active—hoping to go smaller, ideally closer to a C cup. 450/480cc still feels big to me. My breast tissue is in great shape, and I likely don’t need a lift. Is it possible to go smaller than 450cc? I’d like to revisit this with the first surgeon I met (we didn’t discuss size), but he’s booked for months and I need to move forward soon. I’ve also looked into internal mesh bras but am hesitant due to the cancer risks linked to textured material. Would love any input about what a realistic size reduction is.

breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. 
When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.
Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.
Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.
You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals.

Thanks for your question! Without seeing photos, its nearly impossible to make size recommendations. Since your implants now are smooth-walled, I would stay with the same kind of device moving forward. Regarding mesh, it doesn't sound like that would be necessary since we can use your own capsule and tissue to create an internal bra. Good luck!

Thank you for the interesting question for your care and possibly future surgical procedures. Unfortunately did not post any photos so it’s very difficult to give an accurate response or diagnosis. You can downsize and use non-textured mesh. There are companies out there that offer that to give you some peace of mind about the use of textured mesh since your concern of potential Cancer issues with textured mesh. Best of virtual concept with expect fees to range from $10,000-$18,000 again bested virtual consult with.

Without pictures it is difficult to say if you may benefit from a Mastopexy, however, implant replacement using an internal mesh for support does not require the use of textured implants, if such implants are still a concern for you since those associated with Breast Implant-Associated Anaplastic Large Cell Lymphoma have been removed from the market. The available mesh used for internal support have not been linked to breast cancer.
