Hi excellent question that many patients in similar situation can benefit from. First of all you do have a very naturally balanced eyes in terms of your left and right eye and in relationship of the eyes to the top of the nose and sides of your cheek... this balance already provides a pleasant appearance to your face... you do not want to lose this natural beauty for sake of enlarging your eyes and certainly epicanthiplasty will be the worst option because it will create an imbalance as you said and create a permanently visible scar (can range from subtle to very obvious) which will provide a surgerized appearance to what is otherwise a naturally beautiful monolid Asian eye... my recommendation would be to create a small natural fold which tucks in so at a straight gaze, your eyes looks essentially monolid and the fold can only be appreciated when you dynamically close and open or bat your eyes. This procedure will slightly tuck up your eyelid and lashes, slightly enlarging your iris, and thereby creating an appearance of larger eyes with what appears essentially monolid when you gaze yourself in the mirror... epicanthiplasty is an ancillary procedure to create a more satisfactory crease formation in patients whose epicanthal fold would otherwise create an extremely tapered crease formation. If you see nearly equal amount of sclera medial to your iris, as you do lateral, then you have a balanced Asian eye and epicanthiplasty procedure would be an overkill. I hope this helped you and Good Luck!
