Can the collapsed middle portion of my nose be repaired without altering the hump or the tip? Not sure if this is the result of repeated injuries or if it just grew this way. (Note that it's much worse on the right side). Would this be a complicated procedure? Is it worth getting rhinoplasty for such a small change? I know I could get the bump and the tip done and the width taken in and make it more upturned but I'm not interested in having disney princess nose. Can this collapse cause pain?
Answer: Rhinoplasty options You can either have the bump shaved down, which would involve doing a rhinoplasty and moving the bones closer together after the bump is taken care of. You can also have some filler put in which is quite simple and can be done under topical anesthesia, which will hide the contour imperfection, but will need to be repeated from time to time. You need to schedule an in person consultation with a plastic surgeon to discuss this in more detail.

Answer: Nasal evaluation Hello and thank you for your question. Based on your photograph, you may benefit from spreader grafts to help with this contour issue. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and not just a computer animation system. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: Nose shape Hello, Yes it is possible to improve the shape. Without being able to take a full history and not having examined you, my best guess is that you might benefit from a dorsal hump reduction and grafting to the indentation below. In my view this is a realistic reason for surgery and can be achieved. There will be some degree of pain but that can be managed with pain killers.

Answer: Middle vault collapse Dear mdb55,Although you suggest that you have had repeated injuries, you don't really say what those injuries were. They could certainly be the cause of your "collapse". Surgical correction of the collapse would require cartilage grafting. As you have heard, injection of filler materials would provide a temporary improvement and would need repeating every year or so. Good luck!

Answer: Options to repair slightly sunken middle portion of the nose From the limited photographs presented, it appears that the upper lateral cartilages are concave and sunken in. In addition, there is a bony dorsal hump present just above that area. A rhinoplasty procedure can accomplish filing down the dorsal hump and placing spreader grafts composed of the patient's own cartilage underneath the concave upper lateral cartilage in the midportion of the nose. A larger spreader graft will be required for the more collapsed side. This is a rather difficult rhinoplasty, so choose your surgeon very wisely based on extensive experience. For more information about spreader grafts and many before and after examples, please see the link and the video below
