Since the breast shape is pretty good when the muscle is relaxed but forms a groove when you flex your pec, you have animation deformity. This is due to the detached portion of the pectoral muscle adhering to the capsule, right where you see the groove. IMO this is not correctable with fat transfers. It requires re-attachment of the muscle behind the implant, which can be done by converting from dual plane to split muscle. Many examples on my website.

The inferior pocket will need to be released and probably supported with mesh. Fat is not sturdy enough to change the shape, but it can give volume.

In cases like these, the native inframammary fold is persistent and will not yield contour redraping over the implant. In such cases fat grafting with careful needle band release (Rigottomies) is done to attain aesthetic convexification. Best Regards

Your situation is complex and your description is inadequate to understand your situation fully.
The two sets of pictures included show very different outcomes.
To get an accurate assessment, and the ability to give you a quality recommendation would require an in person evaluation with an examination.
From the pictures included I question if fat grafting is going to make a big difference.
It looks like you may have developed a capsular contraction, but this requires a bit more information or an examination to confirm.
I suggest scheduling in person second opinion, consultations with other providers in your community.
For in person, second opinion, consultations, I recommend patience come prepared bringing with them a complete set of proper before and after pictures and copies of previous operative reports.
It may not be a bad idea to simply ask for a complete set of your medical records.
