I had a lift with augmentation/ implant. Surgeon is planning on fat transfer as revision to help with issue. Biggest concern is when flexed or laying flat. Would fat transfer help lower pole fill or new inframmary fold hold better?

You do have a small double bubble. I would suggest wearing a good supporting bra always, especially when exercising. If it does not progress and you are happy, then leave it alone. Otherwise, go visit with your surgeon for this will need surgery to correct.
It appears that you have "bottomed out" likely due to poor incision placement, poor technique, inappropriate implant selection/size/profile, and natural causes from weak tissues at the inframammary fold. You may need some internal support as well such as Galaflex. I would find a surgeon...
