I have grade 3 ptosis so am getting a lift with implants. I dont want a glued on look, but do want a full, perky chest. Im wondering if a high profile always looks glued on or if I could accomodate that without that look? Or if over the muscle would acheive the look Im going for? I dont want a super 'done' look but also dont want to metaphorically go to the salon to pay for a trim- I want tight, perky breasts. My surgeon only does round and under the muscle is dual plane. Please help me figure out the best combo to acheive the dream I have! Im looking at 340 cc round motiva under the muscle mod profile currently but Im nervous theyll turn out not perky enough. My chest is 12 in and not a lot of skin on my chest, but about a b cup of breast tissue.

I think your surgeons plan is excellent and I agree with the size implants and placement.  Some surgeons would do your procedure on two stages. Do the lift first then the implants 3 months later.  It’s easier to give your breasts a full and perky look this way

Based on your description grade III ptosis, a relatively tight skin envelope, and limited native tissue the primary determinant of achieving a full yet natural, “perky” result will be the quality and design of the mastopexy rather than implant profile alone. A properly executed lift repositions the nipple areola complex and reshapes the breast mound, creating the foundation for upper pole fullness. Implant selection should then refine this, not overpower it. High-profile implants do not inherently produce an artificial or “glued-on” appearance; when appropriately matched to your chest width and soft tissue characteristics, they can provide the desired projection without compromising natural contour. Given your measurements, a moderate to moderate-plus profile implant in the 300–350 cc range, placed in a dual plane position, is a sound and commonly preferred approach to balance projection with a smooth upper pole transition. Submuscular placement will also help minimize implant visibility and palpability in patients with thinner tissue. Conversely, over the-muscle placement may increase the risk of a more augmented appearance in your case. Ultimately, precise intraoperative assessment, combined with a well planned lift technique, will be critical in achieving your aesthetic goals while maintaining a natural result.

You would benefit from a breast lift using the Bellesoma. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers or small implants can be placed if additional volume is desired.

It is very possible to achieve a "perky" look with round implants placed under the muscle.  Choosing the correct size and profile will help you in achieving your goal look.  If you are unsure about your surgeon's suggestion, visit with a few other board certified plastic surgeons near you to consider varying opinions.
