Is a lift with implants possible after a free nipple graft? (Photo)

Question

I had a breast reduction 2 years ago. 1100 grams removed and given free nipple grafts. Ive been unhappy with them since the surgery. Im extremely flat on top and bottom heavy with the nipples pulling to the sides. I would love tonknow if i would be able to have a anchor lift and implants. There is no real answer to this on the web

After seeing your photo, I can understand why you are describing a bottom-heavy shape with limited upper-pole fullness. A lift with implants after a prior reduction and free nipple graft can be possible in some patients, but this is a revision breast case rather than a routine augmentation/lift. The main issue is how much safe reshaping can be done through scarred tissue while protecting skin healing and the nipple-areola grafts.
Implants may help upper fullness, but they can also add weight and make shape or healing problems worse if the tissue support is weak. I would expect an in-person exam with a revision breast surgeon to decide whether this is best done in one stage or staged, such as reshaping/lifting first and adding implants later. Ask specifically about the scar pattern, implant size and plane, risk to nipple color/sensation, wound healing, and whether internal support or fat grafting would help your goal.

Revision lift with augmentation post Free Nipple Graft is an option.

Based upon your photos, the NAC is high-riding. As a skin graft, it can be re-grafted. Yet this cannot be accomplished without leaving a scar above the areola which will be visible in most swimsuit/low cut apparel. However, removing lower pole skin and reducing the distance from nipple to fold will create the appearance of a lowered nipple without the additional scarring.

The addition of an implant will fill the upper pole. This can either be done in conjunction or in staged fashion. Often times, revisionary surgery is taken in stages in order to appropriately address each issue without the confounding factors of dual goals.

As always, discuss your concerns with a board certified plastic surgeon (ABPS).

Hi there,Yes, this is definitely possible. Since your nipples have already been grafted and have no sensation, one option would be to re-graft the nipples to a more natural position during that operation. Right now they appear to be quite high-- however, you could also just let them be high-- that part is totally up to you. But adding in an implant and reshaping the breast tissue is definitely possible.I'm happy to help however I can.

Based off your limited photo - there is noted wise pattern scars with pseudoptosis (bottoming out of the lower breast) and high gazing nipples as it seems from the limited angle. With the history of a free nipple graft, you have many options to re-volumize your breast. Implants with re-lift using the same wise pattern is most likely. The nipple needs to be re-sited to a more neutral position that sits more at the apex of your breast instead of above it. But a lot of these assumptions requires physical exam and more photos to fully evaluate. Internal mesh bra can help hold implants up at the IMF (inframammary fold). Fat grafting can be done with a re-lift as well. Seek out a few different opinions prior to making a decision. Good luck.

Yes, a lift with implants is possible. In your case, the curve scar near the fold will need to be redone to shorten the distance between the nipple and the fold. The breast tissue there can be reused to be placed higher on the chest wall and at the same time implants can be placed.

Thank you for the question. Based on your photos, it is possible to place an implant and tighten the skin/breast around the implant. The nipples appear to be in good position.
