Sagging outer upper eyelids. Ready for blepharoplasty? (Photo)

Question

Hi, I am a 57 year old woman and I feel that I am ready for an upper blepharoplasty but I'm not sure if I am ready for a lower one. I feel that I look tired all of the time even after I have had a good night's sleep. As you can see from the pictures I have included, the outer part of my upper eyelids sag quite badly - my right eye worse than my left. Any input is appreciated!

As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. I typically combine this with Ultherapy and thread lifting to lift the brow; RF and skin tightening laser treatments to stimulate collagen production and rejuvenate the skin; and laser or superficial RF devices for improvements in texture, color, and any crepey skin around the eyes as needed. Thank you for your question and photos. It is possible that you’re a candidate for both an upper blepharoplasty procedure as well as a brow lift. These surgeries are often combined to produce more refined results. 
An upper blepharoplasty can reduce and tighten excess skin and modify the fat in this area, while a brow lift is performed to lift drooping lids to define the contour of the eyelid crease. Thank you for your question and photo submitted. Based off your information provided, it seems that you would be a good candidate for an upper blepharoplasty. Though an in-person evaluation is needed, you could also benefit from a browlift with your upper blepharoplasty. This will open up the eye area, remove excess skin, and lift the brow if it has fallen below the brow bone causing additional hooding. IT is important to find a qualified and experienced Board Certified plastic surgeon to do a full evaluation and provide you with a designated treatment plan to reach your desired goals.

You could consider a conservative upper eyelid surgery with emphasis on removing more of the outer skin as you have a deeper crease towards the inner eyelid. Also it would be great to see your whole forehead and see if you would be a candidate for a brow lift. a lateral or temporal brow lift can also reduce the lateral hooding.

Hi there! Blepharoplasty for the upper eyelids can be a great procedure to help rejuvenate the eyes and eliminate a tired look. This procedure can often be performed in the office under mild sedation. Bruising and swelling are expected and results improve over 1-3 months. By performing this procedure alone, I often quote a 75-80% improvement alone. One thing to consider is the contribution of the brow as we age which goes downward - especially to create "hooding" on the sides. The brow lift adds the "icing" to the cake so to speak by pulling the heavy skin above which can't entirely be addressed by blepharoplasty alone. Hope that helps.
