Question

As my surgeon told me that i might continue to bald. He decided to be conservative and only did 1000 grafts for the M shape. Should i go for a second HT in a year time for the crown area?

It can take 6 to 12 months after surgery to see results.  If you are concerned it is best to contact your doctor.  Considering a second surgery right after your first surgery means you probably didn't plan things out properly.

Hello. I think you have a responsible surgeon. His/her advice would certainly be valuable. I guess the question we need answered is what are you doing to prevent further hair loss? Propecia, minoxidil, low level laser and PRP/PRF are all options. If you are on one of these treatments it would be safe to do a small hair transplant for the crown.

I think it was wise to transplant the front first since a good hairline and front will frame your face.   As for the rest, I would give it the year to grow out and see where you are.  In the mean time, hopefully you are on minoxidil or finastride to slow down and improve the quality of your remaining hair. The crown is a large area different from the front where layering of the hair is helpful in giving the appearance of increased density and fullness.  In other words, when the time comes, placing 1000 additions grafts to the crown may not do much and gets lost in the big area.  You would likely need upwards of 2000 to make a real difference. Just something to keep in mind.

I agree with your surgeon that it can be unwise to get too aggressive early on.  As he/she likely discussed with you, grafting the crown can be tricky at a young age before your pattern is fully developed.  You may be better served using things like minoxidil (Rogaine) and laser to help add some density, and consider platelet-rich plasma (PRP) to try to get some regrowth in that area.  Use of finasteride (Propecia) can also help slow down hair loss.

Since you have just transplanted the frontal area, definitely get on some maintenance medications like finasteride or minoxidil to insure your results and prevent future native hair loss. Then wait a full year for the hair to grow in before deciding on a Crown transplant. You might find that filling in the frontal area and being on finasteride and minoxidil has solved your problem since you are such a young patient.

Shock loss can occur at any number of grafts and the first surgery always causes the most shock loss. This can be prevented with the drug finasteride
