Periareolar Mastopexy gone wrong! Please advise me on what can be done - If a full lift is required. (photos)

I had an implant exchange due to PIP&a Periareolar lift 5 years ago. The scarring was so bad around my nipples that I had a revision with another surgeon 2yrs ago with permanent structures. Unfortunately they have spread&the scarring is still bad. I am willing to downsize implants and have a different type of lift- anything that will make them look better than they do currently. If anything my nipples look lower due to them spreading. Please help! I'm willing to travel to get this fixed.
