I’ve had uneven under-eye bags since childhood. When my face is relaxed, it is not very noticeable (albeit you can still see uneven indentions in the left eye). When smiling, it is very noticeable and uneven (reference photo). I understand both fillers and surgery are options, but curious what option would be best considering my age and the long-term benefit.

I would recommend an in-person evaluation with a board certifed plastic or oculoplastic surgeon. In this way they can evaluate the eye as a whole as well as the upper and lower lid function/shape, etc. to advise of options to address your concerns.
