I read John Tebbett’s book Augmentation Mammaplasty and in it the High Five method of tissue-based planning. For “natural” results can you go right up to the BWD with the implant diameter? Or is it optimal to go smaller than BWD? If so, by how much? My BWD was measured at 12cm (but may be closer to 13cm) and I’m trying to decide between Mod+ Xtra implant of 11.5cm diameter (295cc) or 11.9cm diameter (325cc). I’m 36 yrs old 5’8” 165lb. No ptosis. 0 gravida. Good skin quality, no laxity.

Generally using smaller volume, lower profile (also known as projection) is the best way to achieve a more natural appearance. It may also be worth considering using fat transfer (also known as fat grafting) in addition to an implant to add natural volume and customizable cleavage. This helps to soften aesthetic lines and hide the borders of the implant in thin women. It is important to respect the base width diameter, as this is the measure of your breast 'footprint'. Not respecting this and using and implant that is either way to narrow or way to wide can distort your breast aesthetics. In general i think it acceptable to use an implant that measures up to 1 cm less than your base width diameter and up to one half of a centimetre wider and still maintain breast aesthetics.

Answer: Size Your surgeon who has much more knowledge and experience than you can help you select the best fitting implant for your tissue and frame. It is more involved than just selecting the base width. You will be paying for that knowledge.

Answer: Breast Width Diameter and Breast Implants - We commonly place breast implants that are the same width of our breast augmentation patients' base diameter. You can still look natural if you go up to that natural base diameter, or breast width, that you have. - As you might expect, the more you exceed the width beyond your own base diameter, the more "side boob" you'll have after your breast augmentation. - You would look great with either implant size you have listed above. - Best of luck to you, and thanks for sharing!
