Im a 5’3” 120lb woman at 33 years of age who had a breast augmentationabout 1 year and 5 months ago. I started with nothing.28. No children prior and don’t plan to have any. I currently have 325/350cc HP mentor implants and do not like how high they sit and find they’re just too big for my short, thin frame. I have an appointment for a revision to go smaller in February. Curious if going down 100ccs to 225/250 and changing to moderate plus profile will make a big enough difference?
Answer: Size Going down 100 cc's will make you a little less than one cup size smaller. You might even like to go down to just 200 and 225 cc's. Your surgeon can tighten your lateral pockets internally so the implants won't move around too much.

Answer: Big Enough Difference 1 standard liquor shot glass is 44 ounces. Essentially, you will be going down in size by 2 shot glasses. Sometimes going down in implant volume also includes selecting a lower profile implant. High profile implants are not intended for breast augmentation; they were originally created for breast reconstruction procedures. Using a high profile implant for a breast augmentation can create a very unnatural "augmented" look. I would ask your surgeon to potentially also lower the profile to decrease the projection of the implant.

Answer: Breast implant downsizing Dear Modest, Changing the profile and volume of your breast implants is certainly an option. You've got a great result already and it looks like you have nice, youthful skin that will contract down to a smaller volume breast. The numbers you mention are reasonable. Good luck!

Answer: Implant reduction Based on the information you provided reducing your implants by 100 cc, seems appropriate. Lower profile implants will look more natural and maintain the correct diameter to match the current implant pocket. Reducing the implants by 1/3 will reduce the augmentation by 1/3 total volume change from implants. There is no correct yes or no answer to your question. In the end, it depends on what you’re trying to achieve. Personally, I make implant decisions based on the patients reviewing lots and lots of before and after pictures, so I have a clear understanding of what the patient is trying to achieve. I use sizers during surgery and have a large supply of implants available to choose from. This method of choosing implants has worked well for me in the past. I put a significant amount of effort into discussing outcomes and review this over as many visits as it’s necessary for patients and provider to have a clear understanding of what is hoped for in regards to outcomes. Working with before, and after pictures of previous patient, who had very similar body types has been the most consistent and accurate way of choosing implants in my experience. Each provider has their own method of helping patients choose the right, implant, shape and size. Find the right plastic surgeon, and everything should fall into place. Best, Mats Hagstrom, MD
