Jan 2005: 34c implants placed, no complications Jan 2018: left implant replacement due to deflation, no complications Jan 2022: Right breast revision due to sagging/loss of elasticity (implant was still intact). A dissolvable mesh was placed to strengthen skin. No lift was done. October 2023: right breast starting to sag again, left breast seems fuller / higher than right. Question: do I need a breast lift??? I’m 37 years old, 5ft3in 111 lbs

Your lateral pockets can be tightened so the implants sit closer together. If you have donor fat, your surgeon can add this to the inner part of your breasts for improved cleavage. The bases of your breasts cannot be moved.
Read, augmentation, after formal explantation is going to lead to complications and less than ideal outcomes. There will be an inevitable layer of scar tissue that forms contractile bands, potentially distorting the shape of the breast.

I see three issues with your augmentation.
The first is that you have breast divergence. This would be visible on your preoperative pictures, and it cannot be changed with breast augmentation. This is something that you will always have and needs to be excepted.
