I am 21 years old and have tubular breast and I have wanted to get them corrected since I was about 14. I am wondering if it is possible to get an areola reduction and release the constriction of the breast first and wait to get implants later on?
Answer: Tuberous breasts can be corrected without augmentation. Augmentation isn't necessary to correct the tuberous nature of some breasts. By the way, tuberous, not tubular, is the correct term for the deformity as it describes breasts that are shaped like a tuber. In fact, it can be beneficial to do the procedure without the augmentation because the implant puts stress on the periareolar scar tending to make it not heal as finely.

Using only a circumareola incision it is possible to reshape your breast tissue creating upper pole fullness, elevate them higher on the chest wall and more medial to increase your cleavage. At the same time, the size of the areolas can be reduced. If you are satisfied with the volume of your breasts then you do not need implants or fat transfer but if additional volume is desired it can be done simultaneously or later. The ideal treatment for you depends greatly on the degree of each deformity: size of the areolas, how tight the breast tissue is, how constricted the base is, if there is an element of herniated glandular tissue, the texture of of your skin and breast tissue. In some cases I perform a circumareolar mastopexy (Benelli) to reduce the areola and the herniation and reshape the breast first; then at a second stage place implants in. Photographs are helpful: but examination by a board certified surgeon with great deal of experience with these cases is the best way to proceed.

It is best to put the implant in at the initial surgery because the implant helps to improve the shape and I am afraid that placement of the implant at a later date will not give you as good a result.
