Im about to do a revision surgery on a otoplasty performed back in 2019. My pervious surgeon did a good job in creating the the antihelical fold however didnt adress the main cause of protrusion which is excess cartilage on the top part of the ear(triangular fossa).I spoke with the my current surgeon and he said cartilage removal isn’t recommended for the top half of the ear and that removing some skin and placing sutures should produce the desired results.I have attached a picture for reference

While setback otoplasty may not be an option, ear reduction and reshaping surgery may be. Your ear may be surgically reshaped to make them less prominent and reduce protrusion. This can be performed by a board certified plastic surgeon experienced in cosmetic ear surgeon.
Hello there! I can see what you're talking about. A revision can certainly make improvements, but the surgeon would need to be careful in order to not create further deformity or compromise of the skin and cartilage. The soreness within the left ear could be a result of neuropathy or the sutures...
I suggest you go on consults with surgeons who do otoplasty often. It will definitely rely require incisions and permanent sutures if you want it to work
