Hi, I had BA about a year ago, 180 cc HP mentor round silicon implants. My right one settled lower than the left on and is about 1 inch below the incision. My surgeon suggested to have a correction to lower the left one, and change both implants to 280cc - 335cc. My question is whether his decision to lower the left one is better than making the right one higher? I don’t have asymmetry post opt. Will changing to a slightly bigger implants would help with the current problem I have? Thank you
Answer: Breast Revision Surgery Unfortunate to see the issue you are having. Going bigger is really not so much related to the revision of the folds. So, if you want to change your implants size that would be fine. I would urge you and your surgeon to make sure that the larger sizes do fit your body and your measurements. As far as the fold asymmetry goes...you would raise the one side or lower the other it just depends on what your plan is with the increase in size or not. I would say if you stay with the same size, I would personally raise the side that is lower. If you are going bigger then you would need to lower the higher side so the relationship of the NAC to the larger implant is improved. Hope that helps.

But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small.You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. Answer: Implants If you want to go larger, then go with a larger size. The implant (left) that is elevated needs to be brought down. The other side is in a good position. If you are happy with your size, then just lower the raised one.

Answer: Revision Breast Augmentation Your left breast is too high with the nipple areola pointing a bit downwards. The right one is in the proper location. If you desire larger breasts, then entertain increasing the size of your implants at the time of the revision where you should lower the right breast. Best wishes, Dr. Taranow
