Do I need fat grafting to properly correct scar tethering after gynecomastia surgery + revision surgery? (Photo)

Without examining you it is difficult to be certain but based on the photos it looks like a scar release with fat grafting to correct the missing tissue would be the ideal procedure to correct your problem. It is not likely to ever fully correct, but can be significantly improved. The only way to know for sure is to book a consultation with a board certified plastic surgeon near you to see what your options are. Best of luck!  

Thank you for your question and the great photos you provided. I am able to appreciate the tethering you describe in you photos. A minor scar release with Renuva injection may help get you the result you desire. Renuva is "off the shelf" fat; a filler made of an acellular matrix to allow your own body to place its own fat in. Renuva may take up to three months for patients to see final results. Make sure you discuss all the different treatment options. Good luck!

Thank you for sharing your excellent question and photographs.  It appears that the best treatment to help improve upon your left-sided contour irregularity would be scar tissue release with fat grafting.  Liposuction to the lateral chest could also be performed to help remove some of the fullness along the outer chest/underarm region.  Hope this helps.

Thank you for your question.You will benefit from some fat grafting on the left side and also some micro liposuction in some of the areas which have residual fat. Thankyou for your question.Fat injection is a very good option and can you be done under local anaesthesia, however you may need more than one session of the fat injection process.

please realize that overall you have a good result. it appears that fat grafting would be helpful to the indented area on your left breast. I think you should accept the slight difference in nipple protrusion between the 2 sides. There will ALWAYS be difference between the two sides in every operation - whether it be gynecomastia correct, a BA or Breast reduction or a blepharoplasty. The problem is that sometime people become extremely focused on their post op results and really obsess over minor differences between the 2 sides. Slight difference in nipple protrusion are going to happen and i think are impossible to fully correct, unless there is a major difference between the 2 sides.david berman md
