Without a physical exam it's hard to tell for certain, but it does appear that you have some breast tissue pushing your nipples forward.I typically combine the direct excision of the tissue with liposuction contouring of the chest when performing the surgery for Gynecomastia.  I place drains in each side, which stay in for a week. The pain is typically just soreness that patients will take prescription pain pills for 3-5 days.  I place my patients in a surgical garment that is required for 6 weeks unless showering.  I recommend taking a week off of work, and have upper body lifting restrictions for 6 weeks.  The average cost in the Kansas City area is $6000.I hope you find a board certified plastic surgeon to meet your needs.  Best wishes.

Hello!  Thank you for your question and photos.  The surgery to correct your puffy nipples is done while you are asleep.  For a few days afterwords you will experience some soreness but no real pain.  There will be some swelling afterwards but once that has subsided then your chest will be flat.  This should be permanent.  The cost varies from doctor to doctor and from State to State.  Make sure that you a visit a board-certified plastic surgeon who is experienced in this type of surgery so that you get a good result.  Best of luck!
