Thank you for your question and for your photos. This is a common uncomfortable condition that men dread to have. Gynecomastia is an enlargement or swelling of breast tissue in males. It is most commonly caused by male estrogen levels that are too high or are out of balance with testosterone levels. Although there is no cure for Gynecomastia, the only option is to surgically remove the tissue and flatten out the surrounding of the chest with liposuction. The “puffy nipples” you are referring to are a known aspect of gynecomastia, which is the increased presence of male breast tissue. Unfortunately, gynecomastia does not go away on its own since it is resistant to exercise and diet. The good news is that with a simple surgical procedure, the excess tissue is removed, leaving you with a flatter, more masculine chest and flatter nipples. You will be able to go home the same day of surgery, and can return to your full exercise routine in two weeks. It is best to consult with a board certified plastic surgeon with a wealth of experience in male breast tissue reduction.

From your photos you appear to have gynecomastia (which would need to be verified through Skype or an in-office consult), the cause for which is frequently idiopathic, meaning that the cause is unknown in most cases. There are numerous other causes for gynecomastia including anabolic steroids, marijuana, certain prescription medications, liver failure, and certain tumors, among other things. Smaller gynecomastia procedures are performed under local anesthesia and oral sedation. This is a very well tolerated procedure that takes about an hour or less to complete. The patient goes home the same day and begins the recovery period.The larger gynecomastia correction procedure which involves greater tissue removal as well as skin removal is usually performed under general anesthesia. This male reduction procedure takes less than 2 hours to complete, even in the most complicated of cases. The patient goes home the same day.
Puffy nipples is gynecomastia caused by breast tissue behind the nipple. The only way to get rid of it is by surgical excision. Be sure to consult with a board certified plastic surgeon. Good luck.

Thanks for your question. “Puffy nipples” in an otherwise healthy, lean male is usually caused by a small amount of breast gland underneath the areola. If present, this gland has a rubbery texture and would be confirmed with a proper examination by your PS. From your photos it does look like you have minor gynecomastia. Assuming you have no other factors contributing to gynecomastia the only predictable treatment is surgery, a male breast reduction. I perform this procedure, with removal of gland and fat, through a small incision hidden in the armpit. Whichever technique is used, ensure you seek treatment from a PS with expertise in male breast reduction surgery. Good luck.
