Follow-up: Can creased/folded areola complex be corrected after gynecomastia surgery?

Thank you for sharing your question and photographs.  After a year, little improvement will be seen spontaneously and I would recommend reaching out to your surgeon to discuss a possible revision.  Your lower areola may have some residual glandular tissue that can be excised, or scar tissue that can be released.  If the latter, then fat grafting to any areas of deficit or contour irregularity can be done to improve aesthetics and limit the chance of any scar tethering recurrence.  Hope this helps.

Thank you for your question. You may benefit from some Autologous Fat Grafting and possible removal of any residual tissue around the depressed area and also possible removal of scar tissue / bands beneath the nipple. I completely agree with the answer by Dr. Castillo that you probably have some residual tissue, definite traction scarring and maybe some indentation, all correctable by lipotransfer from your hip or tummy, secondary tissue removal with long instuments through a separate site and scar release.  Good luck.Dr. Joe

Hello! Thank you for your question. What you have described is not uncommon after surgery. Gynecomastia can be difficult to get "just right". Based on your post and on your pictures, you may benefit from fat grafting (behind the nipple/areola to partially fill the "empty space" left after your surgery) and scar revision---although not necessarily at the same time. I think it is reasonable to do the following: schedule an appointment with your original plastic surgeon to discuss your concerns and potentially formulate a treatment plan AND schedule an appointment with another plastic surgeon for a second opinion. you need to be seen in person and examined. you may just need the scar tissue released or you may need a small amount of fat grafting done at the same time. you should definitely see your original surgeon about this. No one gets  every result perfect - so simply expresses your concerns and listen to what they suggest. you can always get another opinion, if you don't agree with his/her suggestions.david berman md
