I have had the my gyno surgery an year ago and now my right nipple has a deformity.The bottom aerola looks bit puffy (out from surface) and folded onto the top part. The top half of the aerola looks as if Its attached to the muscle beneath and I can feel a thread like lump right under the nipple when pinched and the nipple is also bit painful when touched. Further the aerola along with bit of its surrounding gets inverted when the hands are raised. Can this complexity be fixed and made normal?

Answer: Gynecomastia Revision? Hello! Thank you for your question. What you have described is not uncommon after surgery. Gynecomastia can be difficult to get "just right". Based on your post and on your pictures, you may benefit from fat grafting (behind the nipple/areola to partially fill the "empty space" left after your surgery) and scar revision---although not necessarily at the same time. I think it is reasonable to do the following: schedule an appointment with your original plastic surgeon to discuss your concerns and potentially formulate a treatment plan AND schedule an appointment with another plastic surgeon for a second opinion. Answer: You may need the scar tissue freed up you need to be seen in person and examined. you may just need the scar tissue released or you may need a small amount of fat grafting done at the same time. you should definitely see your original surgeon about this. No one gets every result perfect - so simply expresses your concerns and listen to what they suggest. you can always get another opinion, if you don't agree with his/her suggestions.david berman md
