Answer: Can fat grafting be done to improve crater deformity? Thank you for sharing your excellent question and photographs. Nothing replaces an in-person examination of your tissues, or knowing how long since surgery you currently are but it appears that much of your tethered scar is at the level of your incision. This may be improved with reopening the incision to release the scar and rearrangement of underlying fat to “gather” material underneath the scar and nipple to improve your outcome. If insufficient fat is present, then grafting could also be performed to help limit the chance of recurrence. As you said, grafting is a bit more unpredictable and may require a series of transfers.

Answer: Fat transfer Dear Forceful6315, It is hard to tell for sure without an examination but based on your photos, it looks like you would benefit from fat transfer to fill the dent. In most cases 30-50% of transferred fat will be lost after some time. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.
