I'm a 19 year old male and I think I may have gynecomastia -- do I need surgery? (Photos)

Gynecomastia or enlarged male breasts is a common problem during adolescence that is supposed to resolve on its own by the time a patient becomes an adult. If the problem persists post 18 years of age, then medical assistance may be required to rectify the issue via medication or surgery.
The physician will first determine the underlying cause, and if the patient has gynecomastia with breast tissue or pseudo-gynecomastia with just fat and proceed with treatment accordingly.

If you have had the puffiness since you were 13, it may well be a case of gynaecomastia. However, it is not a necessary or dangerous condition and unless it is a bother to your esteem you can certainly consult with a board certified plastic surgeon who specializes in  gynaecomastia surgeries.
